What are injectable dermal fillers?

Injectable dermal fillers are injectable gels made from naturally-occurring substances, including collagen and hyaluronic acid. Dermal fillers are injected just under the skin in order to add volume to aging skin, lips and to fill and lift wrinkles and folds. Dermal fillers add volume for a more natural youthful appearance.

The majority of dermal fillers contain hyaluronic acid as their main active ingredient. Hyaluronic acid based dermal fillers include Restylane and Juvederm. Hyaluronic acid is a natural component of the skin.   The hyaluronic acid is a crystal-clear gel called NASHA, or Non-Animal Stabilized Hyaluronic Acid.  Hyaluronic acid is hydrophilic, or "water-loving." This allows it to bind to water to provide long-lasting results.

Injectable dermal fillers are used to fill wrinkles, lines, folds, and scars (eg. acne scars). Dermal fillers are also commonly used to give a fuller appearance to the lips.

The most common areas treated with injectable dermal fillers are the smile lines around the nose and mouth, fine lines above the mouth, frown lines between the eyes, and acne scars.

What Types Of Dermal Fillers Are Available?

Collagen: Originally, dermal fillers were all made of collagen derived from cows. This is referred to as bovine collagen.

Collagen is nature’s most abundant protein polymer. Collagen acts as a scaffolding for our bodies. It controls cell shape and cell differentiation. Collagen is the fibrous protein constituent of skin, cartilage, bone, and other connective tissue. It is the substance that fills the spaces between the cells of the body.

Collagen dermal fillers are not used as often now because of its potential to cause allergic reactions. Skin tests are preformed before treatment with bovine collagen dermal fillers to check for allergic sensitivity.

Today, collagen dermal fillers use human collagen. Cosmoderm, Cosmoplast, Zyderm and Zyplast are examples of human collagen-based dermal fillers still in use today.

Hyaluronic acid: (Restylane,  Restylane Fine Lines, Perlane, Juvederm, Juvederm Plus,) The synthetic hyaluronic acid dermal fillers are the most common fillers in use today. If you see the term NASHA, this stands for Non-Animal Stabilized Hyaluronic Acid. The first hyaluronic acid products were made from rooster comb extracts. In fact, one product Hylaform is still made from rooster combs. The hyaluronic acid products used today are made by genetically copying human hyaluronic acid.

Poly-L-lactic Acid: (Sculptra) Sculptra is derived from fruit acids. It is a biocompatible and biodegradable injectable material known as poly-L-lactic acid that naturally stimulates the body to produce its own collagen.

Calcium Hydroxylapatite: (Radiesse) this product contains calcium hyroxylapatite (CaHA) microspheres. Calcium hydroxylapatite is one of the primary mineral substances of bone. No allergy testing is required. Over a period of weeks following injection your body encapsulates the microspheres with collagen, giving a filling effect. Because the calcium hydroxylapatite microspheres persist for 2-3 years, collagen production is continuous. This product provides the longest duration of all the temporary

How are some dermal fillers different?

Some dermal fillers are hyaluronic acid-based products, such as those made from rooster combs, others are non-animal derived. Non -animal derived fillers virtually eliminates any risk of animal-based disease transmission or allergic reactions such as Restylane®.

What should patients do prior to treatment?

Dermal fillers require no pretesting, but patients should take a few precautions before being treated. Patients should avoid using aspirin, or any non-steroidal anti-inflammatory medication, St. Johns 's Wort, or high doses of vitamin E supplements prior to treatment, because these may increase bruising and bleeding at the injection site.

Are Dermal Fillers safe?

Yes. Dermal fillers are safe. In fact, some products have been used safely by dermatologists and plastic surgeons since 1996 worldwide. When using a non-animal based dermal filler such as Restylane® that is fully biocompatible, allergy testing is not required. Virtually any risk of animal-based disease transmission is eliminated. The most commonly reported adverse events were temporary redness and swelling at the injection site. These effects typically resolve within two to three days.

Can dermal fillers be used anywhere on the face?

Dermal fillers such as Hyaluronic Acid ones like Restylane ®  have been used in more than one million treatments in over 60 countries to correct a variety of wrinkles and to enhance the appearance and fullness of lips. In Canada, these products have multiple indications for reducing fine line and wrinkles and restoring volume for facial rejuvenation. Volume loss is a key sign of aging.  An NASHA filler such as Restylane® adds volume, which may reduce certain signs of aging and help you attain a more youthful appearance.

How long do dermal fillers last?

Studies have shown that the average treatment results last six months, or even longer. The unique technology helps maintain the cosmetic effect until the injected material is nearly gone.

How often should I have treatments with dermal fillers?

Some dermal fillers last twice as long as collagen, so treatments are typically scheduled twice a year. Evidence shows that having a touch-up treatment with the NASHA product of Restylane ® before the product has fully dissipated will enhance the lasting effect.

How is a dermal filler different from Botox®?

Dermal fillers are a natural cosmetic fillers. In contrast, BOTOX Cosmetic® is a drug that blocks nerve transmission and relaxes the underlying muscles. Many patients have been very pleased after receiving treatment with both products.

Do the injections hurt?

Dermal fillers are injected directly into the skin in tiny amounts by an ultrafine needle, resulting in minimal discomfort. The procedure is simple and convenient, and results are practically instantaneous. To optimize your comfort during the short procedure, your physician may anesthetize the treatment area. Dr. Murphy is very gentle and uses a very special technique when freezing the area.  The process is very comfortable.

How much do treatments with dermal fillers cost?
